Certificate of Identity: Loss of Passport

In emergency situations of theft or loss of a passport, a Travel Document can be issued by the Embassy to enable the applicant to return to Cyprus where he/she should submit an application for a new passport.

Application form duly completed and signed

1) Two recent passport-size photographs, at least one notarized by a Notary Public

2) Money order in the amount of the amount of ? CY 10.00 (amount in US $ depends on current exchange rate) made payable to the Accountant-General of the Republic of Cyprus (please note that personal checks are not accepted).

3) Proof of citizenship.

4)In cases of theft/loss of the applicant’s passport an official report from the police authorities should be produced as well as a Statement under Oath, notarized by a Notary Public.

5) Photocopy of a picture I.D. (i.e. your Cypriot I.D. or U.S. Driver’s License)

If application is submitted by mail, for quick and accurate delivery, a self-addressed stamped/ prepaid envelope should be sent. The Embassy bears no responsibility for any loss.
