CERTIFICATES

Applications for certain types of certificates can be made either in person or sent to the Embassy. Please check with the Consulate in advance in order to ensure that you submit the correct documentation.

If you opt to send your application to the Embassy, please include a self-addressed, stamped certified envelope, or pre-paid courier label, for quick and accurate return of your certificate.

Original Birth Certificate

A request for the issuance of an original birth certificate, must be accompanied by:

  • A letter addressed to the competent authorities in Cyprus requesting an original birth certificate.
  • A duly completed Affidavit, signed and certified at the Embassy or nearest Honorary Consulate.
  • Copy of birth certificate issued by the Republic of Cyprus (where applicable).
  • Notarized copies of national passports, US driving license, US visa, US permanent residency card, Cyprus ID (as applicable).

The fee for an original birth certificate is the equivalent of €5.00 in US$ (based on the exchange rate at the time of application).

Affidavit Form

Consular Birth Certificate

A consular birth certificate can be issued to individuals born outside Cyprus after the 16th August 1960 and whose father or mother at the time of their birth was a Cypriot citizen.

The following documents must be submitted for the issuance of a consular birth certificate:

  • A duly completed M121 application form, in duplicate, signed in the presence of a Consular Officer or Honorary Consul. Applications for minors (under the age of 18 years), must be signed by both parents.
  • Original birth certificate. Please note that this will not be returned – we advise applicants to obtain a second original birth certificate for this purpose.
  • Marriage certificate of the applicant’s parents.
  • Copies of both the applicant’s and the applicant’s parents’ passports.
  • Birth certificate or certificate of registration of the Cypriot parent.
  • Where the applicant is a minor, parental consent form signed by both parents.

All applications and accompanying documents must be duly notarized and bear the seal of “APOSTILLE”.

The fee for a consular birth certificate is the equivalent of €28.54 in US$ (based on the exchange rate at the time of application), which includes a stamp to be affixed on the application.

M121 form Statement

Death Certificate

A death certificate can be issued by any District Administration Office, regardless of where the death occurred, upon submission of an application form and a medical death certificate.

In the event of a late registration of death (after the period of one year as specified by law), the following documentation must be submitted:

  • A sworn statement.
  • A duly completed application form.
  • The death certificate.

The fee for a death certificate is the equivalent of €5.00 in US$ and €30.00 for the late registration of death (based on the exchange rate at the time of application).

Certificate of Permanent Residency

A certificate of permanent residency in the US can be issued by the Embassy, typically for those seeking an exemption from military service.

In order to obtain a certificate of permanent residency, the following documents must be submitted:

  • A duly completed and signed application form, which must be certified by a Notary Public.
  • A certified copy of the applicant’s birth certificate.
  • A copy of the applicant’s passport and identification card.
  • Copies of the applicant’s parents’ passports.
  • Evidence of each year of residency in the US (e.g. school reports, Doctor’s statement, applicant’s tax returns (where applicable), parent’s tax returns and/or W-2 forms, utility bills, etc.).

Notes:

  • The Embassy reserves the right to request additional documentation.
  • It is the responsibility of the applicant to keep a copy of the Permanent Residency Certificate for future use.

The fee for a certificate of permanent residency is the equivalent of €9.00 in US$ (based on the exchange rate at the time of application).

Police Record Certificate

An application for a criminal record certificate can be made directly to the Cyprus Police. The application should include:

  • A duly completed application form.
  • An official receipt for the payment of the required fee.
  • Copies of ID and passport for Cypriot citizens.
  • Copies of Alien Book, Pink Slip or Yellow Slip and passport for non-Cypriots.
  • Copies of incorporation of company, partnership, association or other entity or association of persons with legal personality (where applicable).

Payment must be made directly to the Central Bank of Cyprus, in the name of the Chief of Cyprus Police. The fee is €20.00 and can only be accepted in Euro.
