NAME CHANGE

A Statement of Oath must be made by the applicant in order for a change of name to be fully completed, from the entry of the name in the National Registry to all official documents bearing the applicant’s name.

In addition to the Statement of Oath, the following documents must be submitted:

• A letter addressed to the Cyprus authorities requesting an original birth certificate with the new name.
• The current original birth certificate / certificate of citizenship and all official documents issued in Cyprus (passport, ID, driving license).
• Copies of US passport, ID and driving license with new name.
• Any documents proving that family members share the same surname (where applicable).

All certified documents must bear the seal of “APOSTILLE”.

The fee for the name change procedure is the equivalent of €93.54 in US$ (based on the exchange rate at the time of application).

STATEMENT
